Revival Health & Wellness Clinic provides primary care for adults, teens, and children in Salem, Centralia, Mount Vernon, and the surrounding Marion County area. Our approach is straightforward: real time with the provider, careful listening, and a treatment plan you understand.
Crea Rose, FNP-BC is a board-certified Family Nurse Practitioner with Full Practice Authority in Illinois and 15 years of clinical experience. We are accepting new patients of all ages.

WHO WE SEE
Our practice cares for patients across the lifespan: pediatrics from infancy, school-age children, teens, adults, and older adults. Family Nurse Practitioners are trained as full-spectrum primary care providers; in Illinois, that practice scope is recognized through Full Practice Authority.
If your medical needs are best served by a specialist or hospital-based team, we’ll tell you and we’ll help you get there. Primary care is the right fit for most general health needs but it isn’t the right fit for every problem, and we don’t pretend otherwise.
WHAT MAKES A SMALL PRACTICE DIFFERENT
Revival Health & Wellness Clinic is a single-provider practice. That means:
- You see Crea Rose, FNP-BC at every visit. No rotating-providers, no “whoever has an opening.”
- Visits are scheduled with enough time to actually talk through what’s going on.
- Same-week and next-day appointments are usually available.
- Continuity of care is the default, not an aspiration.
The trade-off is scale. If you need a same-day visit on a day we are full, we will refer you to urgent care or your nearest emergency department rather than overbook. We are not equipped for emergency care.
WHAT TO EXPECT AT YOUR FIRST VISIT
- Schedule by phone at (618) 740-2240. New-patient visits are typically 45–60 minutes.
- Bring with you: photo ID, insurance card, a current list of medications and supplements, recent lab results or specialist notes if you have them, and a list of questions you want to cover.
- The visit. We discuss your full medical history, current concerns, family history, and goals. Vital signs and a physical exam are part of most first visits.
- Plan. If labs or imaging are needed, we order them and explain why. If a treatment is recommended, we walk through what it does, what side effects to watch for, and what the alternatives are.
- Follow-up. We schedule the next visit before you leave or send a follow-up via the patient communication channel you prefer.
TELEHEALTH VISITS
Established patients in Illinois can schedule telehealth visits for follow-ups, medication management, chronic disease check-ins, and most non-acute concerns. New-patient visits are best done in person. See our Telehealth page for details.

FREQUENTLY ASKED QUESTIONS
Is a Nurse Practitioner the same as a doctor for primary care?
Family Nurse Practitioners and primary-care physicians are different professions with overlapping scope. In Illinois, FNPs with Full Practice Authority can independently diagnose, prescribe, and manage care across the spectrum of primary care without requiring physician supervision. Crea Rose’s training (Master of Science in Nursing, 2022) and 15 years of nursing experience inform every visit. For complex conditions outside our scope, we refer.
Can you prescribe controlled substances?
Yes, when clinically indicated and within scope. We follow Illinois prescribing rules, the state PMP (Prescription Monitoring Program), and standard-of-care guidelines for controlled substances. We do not run a “pill mill” practice; if a controlled substance is part of your treatment, we’ll have a conversation about goals, monitoring, and safe use.
Do you do walk-ins?
We accept walk-ins when the schedule allows, but appointments are strongly preferred — especially for new patients. Call ahead so we can confirm availability and tell you whether the same day is realistic.
What if I need care after hours?
For after-hours emergencies, call 911 or go to the nearest emergency department. For urgent but non-emergency issues, contact your nearest urgent care or call the clinic the next morning. We are not staffed for 24/7 coverage.
How do I transfer my records here?
You can sign a records release at your first visit and we’ll request your records from your prior provider. If you have key documents (recent labs, imaging, specialist notes, medication list), bring digital or paper copies — that often gets you a more efficient first visit while the formal record transfer is in process.
