Revival Health & Wellness Clinic provides Illinois-required school physicals for students entering pre-K, kindergarten, sixth grade, ninth grade, and any student transferring into an Illinois school from out of state. We also complete physicals for daycare admission, camp, and other school activities that require a clinician’s sign-off.
Walk-ins are not guaranteed during the August back-to-school rush — call 618-740-2240 to schedule, especially in July and August.
WHAT'S INCLUDED
- Full medical history review (parents/guardians are welcome to be present for minors)
- Height, weight, BMI percentile
- Vital signs — blood pressure, pulse, temperature
- Vision and hearing screening
- Physical exam covering head-to-toe systems
- Immunization review and updates as needed
- Completion of the Illinois Department of Public Health Certificate of Child Health Examination form
- Discussion of any concerns about growth, development, nutrition, sleep, mental health, or substance use as age-appropriate
WHAT TO BRING
- Insurance card (or be prepared to discuss cash pricing — school physicals are typically a covered preventive service)
- The school’s blank physical form, if provided
- Immunization records, if available — we can also pull records from I-CARE for most patients
- A list of any current medications
- For students with chronic conditions, a brief written summary of current medications and recent specialist visits
VACCINES
Required vaccines are updated based on grade entry and the most recent CDC recommendations. We can administer vaccines during the same visit when needed, including catch-up doses. Common vaccines reviewed at school physicals include Tdap, MCV4 (meningitis), HPV, MMR, varicella, and the seasonal flu shot when in season.
FOR SPORTS PARTICIPATION
If the student also needs an IHSA sports physical, please mention this at booking. We can complete both physicals during one visit using the appropriate forms.
