Revival Health & Wellness Clinic completes pre-participation physical evaluations for Illinois High School Association (IHSA) athletes, junior high and middle school athletes, club sports, and youth recreational leagues. We use the standard IHSA Pre-Participation Examination Form required by Illinois schools.
Sports physicals are valid for 13 months from the date of exam under IHSA rules. Most families schedule them in late spring or summer ahead of fall sports tryouts.

WHAT THE EXAM COVERS
- Personal and family medical history (cardiac history is reviewed closely — sudden cardiac events in young athletes are the primary safety concern these exams are designed to catch)
- Vital signs and growth measurements
- Vision screening
- Cardiovascular exam — heart and lung auscultation, blood pressure, pulse
- Musculoskeletal evaluation looking for asymmetry, joint instability, or limited range of motion
- Skin exam for active rashes that could spread in close-contact sports
- Review of any current concussion history or recent injuries
- Completion and signature of the IHSA form for the school
IF CONCERNS ARE IDENTIFIED
The vast majority of athletes are cleared for full participation at the same visit. When the exam raises a concern — for example, a heart murmur that needs evaluation, an old joint injury that hasn’t fully rehabilitated, or a family history flag for cardiac arrhythmia — we’ll explain the next step, which may include an EKG, cardiology referral, or sport-specific clearance from a specialist. Our goal is to keep athletes safely on the field, not to disqualify, but we won’t sign off on participation when something genuinely needs to be evaluated first.
WHAT TO BRING
- Insurance card (sports physicals may or may not be covered by insurance depending on plan)
- The school’s IHSA form (if the school provided one) — we also have blank forms on hand
- Information about prior concussions, injuries requiring imaging or specialist care, and current medications
- Family medical history, especially regarding sudden cardiac death in relatives under age 50
FOR STUDENTS WHO ALSO NEED A SCHOOL PHYSICAL
If the student is in pre-K, kindergarten, sixth grade, or ninth grade, or is transferring from out of state, they also need the Illinois school physical on a different form. Mention this at booking and we’ll complete both at one visit.
