Revival Health & Wellness Clinic offers secure video telehealth visits for established Illinois patients. Telehealth is convenient for follow-up visits, medication management, chronic disease check-ins, and many non-urgent concerns — saving you a drive when an in-person visit isn’t necessary.

WHAT TELEHEALTH IS GOOD FOR
- Follow-up visits after lab work or after a treatment plan starts
- Medication management and refill check-ins
- Chronic disease monitoring (diabetes, hypertension, hypothyroidism, anxiety/depression)
- Hormone therapy follow-ups when an in-person exam isn’t needed
- Test result discussions
- Mental health check-ins (depression, anxiety, ADHD medication management within scope)
- Minor concerns where a visual exam is sufficient (rashes, lesions where photos can support the visit)
WHAT TELEHEALTH IS NOT GOOD FOR
- Anything requiring a physical exam — abdominal pain, ear infections, joint injuries, full annual physicals
- Acute or worsening symptoms that may need testing or treatment that day
- Medical emergencies (call 911 or go to an emergency department)
- First visits for new patients in most cases
- DOT, school, and sports physicals — these require in-person exam
- Pellet insertions, injections, IV therapy, and lab draws — by definition in-person
WHO IS ELIGIBLE
Telehealth visits are available for:
- Established patients who have completed at least one in-person visit at Revival Health & Wellness Clinic
- Illinois residents — Illinois licensure governs the visit; Crea Rose, FNP-BC is licensed in Illinois only
If you’re new to the practice, your first visit needs to be in person. After that, you can mix telehealth and in-person visits as appropriate for what’s going on.
WHAT YOU'LL NEED
- A device with a camera and microphone (smartphone, tablet, or computer)
- Reasonable internet connection — the visit is video
- A quiet, private space where you’re comfortable discussing your health
- Your medication list and any home measurements (BP cuff, glucometer, scale, etc.) handy
- For follow-up of specific symptoms — clear photos taken in good lighting if relevant (rash, lesion, injection site)
HOW A VISIT WORKS
- Schedule by phone at (618) 740-2240. We confirm whether telehealth is appropriate for what you want to discuss.
- Receive the link by email or text shortly before the appointment.
- Click to join at your appointment time. The platform we use is HIPAA-compliant and does not require an account or download for most patients.
- Visit length: typically 15–30 minutes depending on what we’re covering.
- After the visit: any prescriptions are sent to your pharmacy electronically; lab orders are sent to the lab of your choice.
PRIVACY AND SECURITY
Telehealth visits use a HIPAA-compliant platform. The visit itself is encrypted in transit and is not recorded. Treat the visit like any clinic visit — find a private space and let us know if anyone else is in the room with you. INSURANCE AND COST
Most major insurance plans cover telehealth visits for primary care services at parity with in-person visits, particularly under post-2020 federal and Illinois state rules. Coverage rules vary by plan, however, and some specialty services may have different policies. We’ll verify coverage when scheduling and inform you about any anticipated cost-sharing.
Self-pay rates for telehealth visits are typically the same as for in-person visits of equivalent complexity.
FREQUENTLY ASKED QUESTIONS
I’m visiting Illinois but live in another state. Can I do telehealth?
Telehealth licensure follows the patient’s location at the time of the visit. As an Illinois-licensed APRN, Crea Rose can see patients located in Illinois. If you’re visiting Illinois and physically in the state during the visit, you may be eligible. If you’re outside Illinois, we cannot provide care via telehealth in most cases.
Can I get prescriptions through telehealth?
Yes for most prescriptions. Controlled substances may have additional rules under federal and state law; we follow current Illinois and DEA telehealth prescribing guidelines and will let you know if your specific medication needs an in-person visit instead.
What if my internet drops or the connection is bad?
If we lose video, we’ll convert to a phone call when allowable. If neither works, we reschedule and don’t charge for the failed connection.
Can my child do telehealth?
Yes for established pediatric patients with a parent or guardian present. Most pediatric concerns are still best handled in person, especially for younger children, but follow-ups and medication check-ins can work well by video.
