WHAT WE OFFER
DOT physicals (commercial driver medical exam)
Required by the Federal Motor Carrier Safety Administration (FMCSA) for anyone holding a commercial driver’s license (CDL) operating in interstate commerce. The exam follows the FMCSA medical certification standards and results in a Medical Examiner’s Certificate (the “DOT card”) if you pass.
What’s checked: vision (20/40 each eye, with or without correction), hearing, blood pressure (limit: 140/90 for full 2-year certification, with conditional categories above), urinalysis (protein, blood, glucose), heart and lung exam, neurological screen, and review of medical history including diabetes management, cardiovascular history, and medications that could affect driving.
Bring with you:
- Photo ID and your CDL or CDL permit
- Current medication list (names and doses)
- Glasses, contacts, or hearing aids if you use them
- Any specialist letters relevant to your medical history (cardiologist letter for heart conditions, sleep study results if you have OSA, A1c if you have diabetes, etc.)
School physicals
Required by Illinois schools for students entering kindergarten, sixth grade, and ninth grade, plus periodic well-child checks. We complete the standard Illinois Certificate of Child Health Examination, including vision and hearing screening, growth measurements, and immunization review.
Bring with you:
- Your child’s current immunization record
- The school physical form provided by the district (we’ll fill in the medical sections)
- Any specialist notes if your child has chronic conditions (asthma plan, allergy plan, ADHD documentation)
Sports physicals (preparticipation evaluation)
Required for student athletes participating in school or organized league sports. Follows the IHSA preparticipation form. Includes cardiovascular risk screening, musculoskeletal exam, and review of any prior injuries.
We are familiar with the IHSA form and red-flag conditions (family history of sudden cardiac death, undiagnosed exertional symptoms, concussion history) that require additional evaluation before clearance.
WHAT TO EXPECT

- Visit length: DOT physicals run 30–45 minutes. School and sports physicals are usually 20–30 minutes.
- Same-day result for school and sports physicals. DOT certification is issued at the visit if you pass; conditional results (e.g., requiring a sleep study or specialist clearance) trigger a follow-up plan.
COMMON REASONS TO SCHEDULE EARLY
- DOT card expiring soon — check the date; plan ahead because conditional certifications may need additional documentation
- School physical deadline before the start of the school year (most districts require it before classes begin)
- Sports tryouts or first practice (most schools require the physical on file before participation)
- New CDL applicants — certification before testing
COST AND INSURANCE
School and sports physicals are often covered by insurance as preventive care, depending on your plan. DOT physicals are typically self-pay because employer or driver requirement is not a covered medical benefit. Self-pay pricing is provided when you schedule.
FREQUENTLY ASKED QUESTIONS
How long is a DOT physical valid?
The Medical Examiner’s Certificate is valid up to 24 months. If you have certain conditions (controlled hypertension, diabetes managed by insulin, etc.), the certificate may be issued for shorter periods — typically 12 months or less. The duration is the medical examiner’s clinical decision based on FMCSA guidelines.
I take blood pressure medication. Can I still pass DOT?
Yes. Hypertension is one of the most common conditions DOT examiners manage. As long as your blood pressure is controlled within FMCSA categories, you can be certified. Bring your medication list and recent BP readings if you have them.
What about diabetes?
Drivers with insulin-treated diabetes can be certified under FMCSA’s diabetes exemption process. We’ll review your A1c, current management, and any history of severe hypoglycemia. You’ll need a treating-clinician letter and ongoing documentation.
What if I fail the DOT exam?
Most “failures” are conditional — meaning we identified an issue that needs follow-up before certification (uncontrolled BP, possible sleep apnea, vision below threshold, a cardiac history that needs cardiologist review). We’ll explain exactly what’s needed, write the letter you can take to the relevant specialist, and schedule a follow-up to recheck.
Can my child get all required school forms done at one visit?
Yes. School physical, sports physical, and a vision/hearing screen can usually be combined into a single visit. Tell us at scheduling so we can plan the time.
