What information we collect
From website visitors: standard web-server logs (IP address, user agent, pages visited, referrer) collected by our hosting provider for security and performance purposes. We do not run advertising trackers or sell visitor data.
From patients: the protected health information (PHI) you provide during clinical visits, including history, current concerns, lab results, medication lists, and insurance information. PHI is handled in accordance with the Health Insurance Portability and Accountability Act (HIPAA). Patients receive our separate Notice of Privacy Practices (NPP) at intake, which governs PHI use and disclosure.
From email and phone inquiries: your contact information and the substance of your inquiry. We use this only to respond to you.
